We maintain medical records to help serve you and your health care providers, and to meet legal, accrediting and regulatory requirements. Release of information from a medical record must meet legal requirements. You, or those who are legally permitted to do so, can request access to your protected health information at any time.

To request a copy of your medical records, you must complete our Release of Information form. This form must be filled out completely in order to be processed in a timely manner. All fields must be completed, including the specially protected health information section.

Previous / Current Patients - If you are requesting your medical records for personal use, please note that there is a fee for the requested records. Please see the Oregon Law of Medical Records fee schedule for more information.

*Note: Our Medical Records Department has 30 days from the date we receive the signed release to process the request and we limit release of records to the most recent 2-year history. 

This release allows our office to speak with family members, friends, a significant other, etc. regarding your medical care. If you would like to allow permission for our office to speak with someone regarding your medical care, this form must be completed and in your file in order to do so.

View your health information online at Patient Fusion. If you have not yet signed up for a Patient Fusion account, please contact our office for information on how to sign up.

This release allows our office to request records from your previous Primary Care Provider, specialty provider(s), etc. This form must be filled out completely in order to be processed in a timely manner. All fields must be completed, including the specially protected health information section.

New Patients - If you have a previous PCP and would like our office to obtain those records, please complete this form and return it to our office.
