Cancellation / No-Show Policy
Our Cancellation and No Show Policy is that we require a 24-hour notice prior to any cancellations or rescheduling of appointments. If we do not receive a 24-hour notice, the appointment will be marked as a no-show and you, the patient, may be subject to a no-show fee. For more information on this policy, please click here.

Prescription Refill Policy
Our Prescription Refill Policy is as follows:
If you are in need of a refill of a non-controlled medication, please contact your pharmacy to have them initiate the request. Once we receive their request, we will respond within one (1) business day. If you have not heard from your pharmacy or our office within two (2) business days, please call our prescription refill line at (541) 758-0766 ext. 110 to check on the status of your refill. There is a chance we may not have received the request from your pharmacy.
If you are in need of a refill of a controlled medication, please contact our office to schedule an appointment as ALL controlled medications must be refilled in an office visit, unless otherwise indicated by your provider.
If you are in need of a new/temporary prescription (i.e., antibiotics), please contact our office to schedule an appointment as we cannot prescribe ANY new/temporary medications without an office visit.
